What you may need

  • Photo ID or another identity document
  • Proof of SNAP, Medicaid, SSI, Veterans Pension, Federal Public Housing Assistance, or another Lifeline-qualifying program
  • Income proof if you qualify by household income instead of a benefit program
  • Current residential address and last four digits of your Social Security number or Tribal ID
